Purpose
This document acknowledges the storage of Platelet-Rich Plasma (PRP) prepared for the above-named horse(s) and outlines the limitations of responsibility regarding its storage.
Storage Acknowledgment
At the request of the client, Paddock Equine Veterinary agrees to store PRP in its pharmacy freezer as a courtesy only. The client understands and agrees that this storage is provided without guarantee as to the continued integrity, viability, or effectiveness of the PRP.
Limitation of Liability
The client acknowledges that Paddock Equine Veterinary shall not be held responsible or liable for any loss, damage, degradation, contamination, or reduced effectiveness of PRP resulting from circumstances beyond our control, including but not limited to power outages, electrical failures, freezer or equipment malfunction, acts of God (including fire, flood, storms, or natural disasters), temperature fluctuations, mechanical or system failure, or any other unforeseen event.
Assumption of Risk
The client voluntarily assumes all risks associated with the storage of PRP, including the risk that the PRP may become unusable for treatment.
Release of Claims
The client hereby releases and holds harmless Paddock Equine Veterinary, its owners, veterinarians, employees, and agents from any and all claims, demands, damages, or causes of action arising from or related to the storage, condition, or use of the PRP.
No Guarantee
No guarantee is made regarding the shelf life, sterility, or clinical effectiveness of PRP stored at Paddock Equine Veterinary.
Acknowledgment
By signing below, the client confirms that they have read, understood, and voluntarily agree to the terms of this waiver.
